By submitting this form, you are agreeing to begin the process of starting a merchant agreement.

This form is the preliminary start to the process. A formal agreement will be sent once our advertising department has made contact with you. Mission Discount Cards utilizes a digital signing service to make this process convenient for you.

There is no cost to you. The agreement is a document that we’ll have on file stating you will honor the following:

• Merchant will honor the offer until the card’s expiration date.
• Merchant shall receive complimentary discount cards for their contributions.
• By agreeing to participate in this community event stated discount will be honored for 12 months.
• The cards are good for one use per day.
• Offers on the card are not good with any other offer, promotion, or discounts.
• The discounts are only good at the locations written on the agreement.

By submitting this information, you give Mission Discount Cards permission to contact you to finalize the agreement.

Mission Discount Cards values your privacy and will never give your information away to third parties.

There is NO Charge to the business for participation. Mission Discount Cards reserves the right to typeset any business whose logo in not satisfactory print quality.
